For our first flight we are boarding A43 and A44. Our connecting flight which is long we are boarding B20 and B21. do you think we have a good chance of sitting together. Does the number mean there are 43 people ahead of me boarding?

On southwest, yes at most. The first 15 are reserved for A+ Rewards members. There usually are not that many on the flight so typically you don't have to worry that much. you can go to the desk and ask for early boarding due to health reasons if you are comfortable doing that. Definitely go ask for early boarding after surgery so you can get a seat with leg room. You will need it.

You'll be fine with an A or B, no worries or need to pay extra. The only concern is with the B group carry on space may be filled up, so I'd check your bag.
Is that because you are flying to Mexico? I am confused. I have flown a lot and that number combination sounds like seat/row. A 43 sounds like seat A in row 43. So it would appear to me that you are sitting one in front of the other. Just my two cents and I have flown a lot. Never to Mexico but tons in the US and overseas to Europe and Asia. Good luck!
Even with early check in, you will be behind all the medical assist and families able to pre-board. I have asked for pre-board medical several times especially right after knee replacement surgery at age 45. You just ask the counter person at the gate for a medical pass, they don't usually ask why but you can always just say you had knee surgery and lack range of motion or need to stretch frequently if they do.
Don't worry about paying for early boarding.....its just a way to rip you off. And it means you will be sitting in those little airplane seats all the longer. Stay standing and walking around the gate area as long as possible. Board as last as you can to minimize sitting time, especially postop when blood clots are an issue. Get an aisle seat if possible on the return flight so you can get up and walk periodically. Wear your compression stockings on the return trip.
I was just reading about the storm hitting California. Maybe a good thing to postpone.
