    call that doctor and make an appointment then call your PCP

    Research the surgeons you would like to have and call to see if they take your insurance & then make the Initial appoitment (it's not a 100% commitment to surgery it's just a consultation ~ the surgeon WILL know all the steps you need to take to meet your insurance qualifications
