Hello ,

I spoke to my insurance and they stated weight watchers is coveted as long as I have photocopies of the stamps and or written proof of participation for six months AND 2 physicians visits during the time period. My question is how does all of this get documented with my physician? I told her I’m starting weight watchers and I’m thinking about gastric sleeve and that’s about it. I do not want any hiccups because I plan on getting my surgery in March . Do you have any suggestions? Also they gave me the sheet attached I’m so confused on how to fill this out please help if you can

It appears that the form must be signed off by your doctor. The purpose is to document 6 months of medically supervised weight loss. You must see the doctor once a month for the 6 months. IF you try Weight Watchers, they want a copy of the weekly stamp you get if you go in person. So I would make a few copies of the form. Keep 1 with you if you are going to weight watchers or any one else. Give 1 copy to your doctor to keep on file for those monthly visits.When it is done, it gets submitted to the insurance company.

To be safe I would confirm with your insurance exactly what you have to do. I would ask for it in writing so that you can ensure you follow all the rules.
