Check it out. They do cover the procedure, but i have also noticed that bcbs requires a history of medical issues that may go a year or two back before considering letting you have the surgery. They also require you to lose a percentage of weight on your own. If you dont the will deny you and if you fall under a 40bmi you will need 2 co morbities. Sleep apnea, high blood pressure, diabeties. Theres a few more. Should be on that link. Best of wishes.
I have BCBS and live in Oklahoma and was approved with no 'diet history' (although I have a couple of decades of 'diet history') or additonal requirements outside of psych clearance, NUT visits(3), physical therapy consult, and meeting either 40 BMI(which I excelled at) OR 35 bmi with co-morbidities. The way that it was explained to me is that each employer chooses whether they will offer additional coverage options, such as bariatric surgery or infertility treatments. So, just because I am insured through BCBS of OK and mine was a covered procedure, it was only covered because my hubby's employer choose to cover that procedure. Someone else with BCBS of OK whose employer does not choose to offer coverage for bariatric procedures will be denied.
