From what my surgeon said (the good one in Taipei) if you have only reflux and no hernia, only a bypass will work.
If you have a big hiatal hernia (sup to 3 Cm, min is 4) then you can have a repair of the hiatus and hernia. This is a complicated surgery and my last surgeon (in Paris) chickened out.
One of the name for the repair is Hill gastropexy, or Hill repair, they could use some mesh also during the repair if the hiatus is too big, depending on the damages.
It is more technically challenging than a bypass.And not many surgeons do it.
Also, there is several types of bypass, depending on the length of intestine they take off non absorbtion could be increased or decreased.
