BMI 37.4. Not sure insurance will cover surgery. Freaking out!

I am just starting the 6th month program, as required by the insurance, however, I cannot get an approval until I have completed the 6 months. I have been battling my weight my entire life. 2 years ago, I was above 40 BMI. Lost 50 pounds and gained 32 back. Story of my life! I have to pay about $1500 up front for the program and I may not get approved for the surgery. The insurance company nor the drs office will give me any real answers. I am really freaking out.
