Sooo frustrated!! Insurance companies UGHHH!!!

So I started my journey October 31, 2012 and I have United Healthcare through my husband's employer. I find out the surgery is covered, find a surgeon and surgeon's office calls and is told that surgery isn't covered, so I told her yes it is so she calls back speaks to a supervisor and is told it IS covered, gets all the requirements etc, I go along and do my visits with my surgeon, psych eval, sleep study, EGD, nutrition visits, complete everything the INSURANCE company SAYS is required only for
