Exclamation Folder documenting journey - can't find it thru search

There is someone on the forum who had this nifty idea for a folder to collate all the details of the Sleeve journey.
I've tried looking for it through searching, but no luck.
Lots of dividers recording weight loss, food likes/dislikes, paperwork....
    Is anyone able to help?
    Highest weight ever - 108kgs, Surgery weight -101kgs, Goal weight - 65kgs

I think it was actual paper. She was filing the paperwork and handouts from her medical team, along with other stuff.
I will look at something for the iPad too.

I use a program called "Evernote" that's let's me save photos and notes and thoughts on my phone but also let's me organize them on my computer. It's setup like a collection of note/scrap books.

I just journal the old fashioned way! I am computer illiterate so I have to have paper back-up! . I did this when I quit smoking a few years ago and it helped with the nicotine cravings and hoping it works for my "head" hunger after surgery. It seems to be helping with my pre-op diet, down 12 pounds prior to surgery, I'll take that!!!
